Request Access to Start Using the Provider Benefit Tool

Access to secure member information via the Provider Benefit Tool is only available to providers and facilities contracted with SBA. Non-contracted providers can call Member Services at 1-800-207-1018 for benefits, eligibility, and claims information.

Instructions for SBA Participating Providers:

Get a Secure Access Login

To get a secure access login, you will need to complete and submit BOTH* of these documents:

Email all completed documentation to providerwebservices@selecthealth.org.

*Note: If you are adding a user to an existing account, you only need to submit the login application.

Please Note: SBA's Provider Benefit Tool is administered by Select Health. You will see a Select Health logo in the tool. This is normal.
